WELCOME TO EAST FLORA ELEMENTARY SCHOOL!
We are excited for your child to enroll here at East Flora Elementary School! Outlined below are the documents required to enroll new students.
REQUIRED DOCUMENTS FOR NEW STUDENT ENROLLMENT:
- Withdrawal form from prior school
- Discipline record from prior school
- Mississippi Immunization Form 121 (original from your doctor’s office)
- Birth Certificate
- Social Security Card (copy)
- Custody Papers (if parents are divorced or parent not listed on the birth certificate)
- Proof of residency (copy of mortgage document, property deed, or current lease agreement)
- One (1) current utility bill in the parent’s name showing the street address and dated within the last 60 days (water, gas, electric, cable/internet) *no cellphone bills accepted
- Completed registration form
A “Special Affidavit of Residence” is required when a student resides with their parent or guardian in a home or apartment not owned or leased in the name of the parent or guardian. To learn more about the Special Affidavit of Residence process, visit the Madison County School Webpage.
